概要

現在、学内は吉賀康裕(H8)をチーフ、福江宣子(H11)、大野誠（H15）福田昌和(H18)、石口博智(H22)、藤井翔平 (H26)、久岡雅弘(H28)、橋本慎太郎(H30)また保健学科助教の小室拓也(検査技師)で構成されています。不整脈治療の黎明期から長い間、清水昭彦先生(S54)が不整脈グループを牽引してこられましたが、平成30年より宇部興産中央病院院長・その後顧問に就任され、新たに吉賀がチーフとなりカテーテルアブレーション・植込み型デバイスを中心に診療および研究を行っています。 特に心房細動に対するカテーテルアブレーションは、吉賀が留学先のドイツハンブルクの Asklepios Klinik St. Georgの K-H Kuck教授のラボへの留学経験を活かし、臨床および研究を行っています。 近年発展の目覚ましいデバイス領域においては、福田を中心に遠隔モニタリング、着用型自動除細動器、皮下植込み型除細動器、リードレスペースメーカを積極的に取り入れ、経静脈リード抜去の認定施設としてデバイス診療のすべての領域をカバーできるように診療に取り組んでいます。 石口はR5年度からLiverpool大学、Liverpool Centre for Cardiovascular Scienceへ留学中であり精力的に情報発信に努めています。 また、大野誠(H15)がUniversity of Kentuckyに留学後帰学し今後の細胞生理や概日リズムを中心とした基礎研究からのアプローチを強化する予定です。また福江は日本循環器学会女性循環器医コンソーシアムコアメンバー、支部ダイバーシティ推進委員として活躍し、循環器のみならず医療の分野でのダイバーシティや働き方改革、循環器医の未来に向けた活動で日本をリードしています。
学外においては不整脈グループ出身者（清水・山縣・立野・江里・上山・早野・田村・大村・板垣・木村・角川・土居・金本・亀谷・沢・鈴木・杉・大宮・吉田・平塚・加藤・文本・内田）が 山口県内のみならず西日本の各地で活躍しています。

研究内容

心房細動に対するカテーテルアブレーションにおいて、コンタクトフォースカテーテル、クライオバルーン、ホットバルーン、レーザーバルーンといった新規デバイスの成績・特徴・合併症等を評価し、また心房細動に対する抗凝固療法、心機能との関連についても吉賀を中心に研究しています。 特にクライオバルーンやホットバルーンは使用成績調査施設に選定され、中国地方において最も早期に導入されています。 持続性心房細動に対するカテーテルアブレーションの成績、治療ストラテジーの構築、リモデリング抑制、血栓形成との関連等は吉賀・小室を中心に、また上大静脈と心房細動との関連は吉賀・久岡を中心に、長期予後との関連は吉賀・石口を中心に心房細動診療に関して総合的に研究しています。また突然死や遺伝性不整脈、 デバイス治療、遠隔モニタリング等に関して福田を中心に研究を行っています。

診療の特徴

心房細動に対するカテーテルアブレーションを得意分野としています。さらに最新のバルーンテクノロジー デバイスを用いた心房細動アブレーション、CARTO、EnSite NavX、Rhythmiaといった3Dナビゲーションの最新バージョンを駆使して、難治性頻脈性不整脈の治療にあたっています。 難治性心室頻拍においても心内膜・心外膜アプローチによりアブレーション治療を行うことが可能です。デバイス治療においてはペースメーカ治療に加え、植え込み型除細動器、心臓再同期療法、刺激伝導系ペーシング、リードレスペースメーカ、 着用型自動除細動器(LifeVest)、皮下植込み型除細動器(S-ICD)を用いて治療を行っています。 心臓植込みデバイスでは最適なリードマネジメントに対応すべく、県内で唯一の経静脈リード抜去認定施設としてリード・デバイスマネジメントをトータルに行えるような体制となっています。 さらに自宅に居ながらにして生体情報を管理する遠隔モニタリングを導入して診療にあたっています。このように近年発展の目覚ましい不整脈治療領域ほぼ全ての分野において最先端の治療を提供できる体制を整えています。
